Creating a New webpage with a Contact Form
- Create a webpage with new content.
- Example: https://healthcare.pilotfishtechnology.com/population-health-integration-case-study
- Create URL to include important keywords
- Label all images on the page with internal Alt tags that contain keywords and detailed descriptions
- Create H1 title tag & H2 tags for subtitles
- Create <title> tag to be used in SEO – don’t use default – use a human
- Create <description> to be used in SEO – don’t use default – use a human
- Create internal links from other pages to this page
- Create internal links to other pages from this page
- Add page to the website navigation
- Check the spelling & grammar on the page
- Create Form for webpage – If the bottom of the page contains a button like “Request Custom WebEx” or “Request Free Use Case Evaluation” then create a second web page with a form for entering contact information.
- Add the same image that is on the main content web page to the top of the form page.
- Make the URL the same as the main page but add the word “form” to the end.

- Make the page “no index, no follow”, check “Exclude from Search Results” & “Exclude from HTML Sitemap” because it should not be in any sitemap or crawled by robots.

- Create a Thank You webpage – with the same URL and the words “thank-you” as the prefix.

- In Google Adwords, go to Tools, and then Conversion Actions
- Add a new conversion by clicking on the + conversion button
- Select “Website” as the source of the conversion
- Enter Form – xx – URL – where xx = next number available and url=the last section of the URL from the thank you page
- Example: thank-you-population-health-integration-case-study
- No value for the conversion.
- Count ONE conversion
- Conversion Window – 90 days from ad clicks & 30 days for view-through conversion window
- Category = Lead
- Include in Conversions = Yes
- Attribution Model = Use “time-decay model”
- Click Save and continue button
- Add the Form to Google Tag Manager
- Click on “healthcare.pilotfishtechnology.com” or appropriate container name
- ??? Add tag
- Add Goal to Google Analytics
- Add Form to WordPress Form Conversion Counter???? whatever that is called
- Create Email to be sent out when form submitted to list of people
- Make sure email lists the correct form it is from
- Test the Form Request
- Make sure email is received by list of people correctly
- Make sure GTM is firing correctly using Google Tag Assistant
