Creating a New webpage with a Contact Form

  1. Create a webpage with new content.
      1. Example: https://healthcare.pilotfishtechnology.com/population-health-integration-case-study
      2. Create URL to include important keywords
      3. Label all images on the page with internal Alt tags that contain keywords and detailed descriptions
      4. Create H1 title tag & H2 tags for subtitles
      5. Create <title> tag to be used in SEO – don’t use default – use a human
      6. Create <description> to be used in SEO – don’t use default – use a human
      7. Create internal links from other pages to this page
      8. Create internal links to other pages from this page
      9. Add page to the website navigation
      10. Check the spelling & grammar on the page
  2. Create Form for webpage – If the bottom of the page contains a button like “Request Custom WebEx” or “Request Free Use Case Evaluation” then create a second web page with a form for entering contact information.
    1. Add the same image that is on the main content web page to the top of the form page.
    2. Make the URL the same as the main page but add the word “form” to the end.
      1. Example: https://healthcare.pilotfishtechnology.com/population-health-integration-case-study-form
      2. Make the page “no index, no follow”, check “Exclude from Search Results” & “Exclude from HTML Sitemap” because it should not be in any sitemap or crawled by robots.
      3. Create H1 title tag
      4. Label all images with internal Alt tags
      5. Page will not appear in the sitemaps or organic results & therefore does not need SEO optimization
      6. Check the webpage spelling and grammar
  3. Create a Thank You webpage – with the same URL and the words “thank-you” as the prefix.
      1. Use the same URL and the words “thank-you” as the prefix.Example: https://healthcare.pilotfishtechnology.com/thank-you-population-health-integration-case-study
      2. Make the page “no index, no follow”, check “Exclude from Search Results” & “Exclude from HTML Sitemap” because it should not be in any sitemap or crawled by robots.

  4. In Google Adwords, go to Tools, and then Conversion Actions
      1. Add a new conversion by clicking on the + conversion button
      2. Select “Website” as the source of the conversion
        1. Enter Form – xx – URL – where xx = next number available and url=the last section of the URL from the thank you page
        2. Example: thank-you-population-health-integration-case-study
        3. No value for the conversion.
        4. Count ONE conversion
        5. Conversion Window – 90 days from ad clicks & 30 days for view-through conversion window
        6. Category = Lead
        7. Include in Conversions = Yes
        8. Attribution Model = Use “time-decay model”
        9. Click Save and continue button
        1. Add the Form to Google Tag Manager
          1. Click on “healthcare.pilotfishtechnology.com” or appropriate container name
          2. ??? Add tag
        2. Add Goal to Google Analytics
        3. Add Form to WordPress Form Conversion Counter???? whatever that is called
        4. Create Email to be sent out when form submitted to list of people
          1. Make sure email lists the correct form it is from
        5. Test the Form Request
          1. Make sure email is received by list of people correctly
          2. Make sure GTM is firing correctly using Google Tag Assistant
